Back to SearchPoint Edward, ON - Age-Friendly Sarnia and Lambton Public Health have launched a new online resource to help community members navigate health and social support systems in Lambton County: agefriendlysarnialambton.ca.
The website, funded by the Ontario Health and Well-Being Grant Program, lists more than 200 community programs and services available to help adults transition through the aging process.
"We heard from many community partners that information access was a challenge," said Anita Trusler, Health Promotion Supervisor at Lambton Public Health. "Ensuring all available services in Lambton County are listed on one website will help community members find the supports they need to age in place for as long as they can."
Strengthening and building community support and health services was an identified priority in the Age-Friendly Sarnia Community Action Plan. Priorities included an integrated community care model for Sarnia-Lambton linking health and social sectors to support seniors, who are aging in place.
Lambton Public Health and Age-Friendly Sarnia have joined forces with community partners on a number of projects to help Sarnia-Lambton be an age-friendly community. Healthy aging is one of the strategic priorities identified by Lambton Public Health for 2014-2019.
Opportunities to receive hands-on support and learn how to navigate the website will be available at the Age-Friendly Sarnia Expo taking place June 13th from 10 a.m. to 7 p.m. at the Sarnia Arena.
