Back to SearchPoint Edward, ON - Lambton County residents can now access home and community care supports they need to remain independent, supported and healthy at home thanks to Caredove, a new online referral management network.
Lambton Public Health (LPH) and Lambton Elderly Outreach (LEO), in partnership with Caredove, launched the referral network to streamline access to community supports and health services that allow residents to "age-in-place."
"The best care keeps people at home, safe and independent," says Anita Trusler, Supervisor of Health Promotion at Lambton Public Health. "Traditional referral processes weren't working. Caredove improves the transition between services and provides better access to community service, when and where they need it."
Caredove securely and seamlessly simplifies health and social service referrals to connect people with community supports. Primary care providers can also search for services and send electronic referrals with immediate notification changes to help reduce patient wait times.
