Building an integrated community care model for Sarnia-Lambton

Sarnia, ON - Thanks to a $591,300 grant from the Ontario Health and Well-Being Grant Program, Lambton Public Health (County of Lambton), and its community partners are planning to create an age-friendly community.

The two-year project will strengthen and build community support and health services identified in the Age-Friendly Sarnia Community Action Plan. Priorities include an integrated community care model for Sarnia-Lambton linking health and social sectors to support seniors, who are aging in place.

"This is an important first step," said Anita Trusler, Health Promotion Supervisor at Lambton Public Health. "Funding will enable us to work with our community support agencies to enhance connections between the various health and social sectors."

An age-friendly community supports active, healthy and engaged living for people through all life stages and ability, and fosters civic, economic and social participation.
