Petrolia, ON - The innovative Lambton Community Paramedic Pilot Program has completed its 1,000th home visit since the program launched in August 2017.
Preliminary results indicate the program has resulted in significant reductions to local Emergency Department (ED) visits as well as a decrease in 9-1-1 calls.
The goal of the program is to proactively provide care to patients who are high users of 9-1-1 and the Emergency Departments in their communities.
"Reducing the number of 9-1-1 calls and ED visits improves access to community services and improves the wellness of individual patients," said Steve Pancino, Manager, Emergency Medical Services, County of Lambton. "As the local population ages, programs such as this help keep vulnerable residents safe while reducing costs throughout the healthcare system."
“Bluewater Health is pleased to partner with the County of Lambton’s Emergency Medical Services Department and local Primary Care providers on this innovative, and very valuable program,” said Laurie Zimmer, Vice President Operations, Bluewater Health, “Patients with complex needs receive the proactive care they need through regularly scheduled visits from the Community Paramedics, who work in consultation with the patient’s Primary Care team. This approach has resulted in better health and quality of life for patients with fewer ED visits and hospital admissions.”
The County's Emergency Medical Services Department continues to work with Bluewater Health to evaluate the effectiveness of the program.
