The County of Lambton Social Services Division offers assertive community outreach through two community outreach workers. This team works closely with unsheltered households experiencing homelessness, with the focus of progressing them directly into more permanent housing. Through trauma-informed care and client-centered housing case management, the most vulnerable members of our community can receive supports in-place where they may be residing.
The homelessness outreach team works in close concert with partner agencies such as Lambton Public Health, the North Lambton Community Health Centre, and Sarnia Police Service IMPACT. This multi-disciplinary and collaborative approach seeks to lower barriers, improve coordination of care, and offer more responsive engagements. Through this joint effort, households are better connected to primary care services, harm reduction tools and services, mental health and addictions services and housing solutions.
